Community Resource Coordination & Outreach Team

Our Community Resource Coordination + Outreach Team of community health workers provides care coordination and outreach to individuals and families. We can assist residents to find solutions to longer-term challenges such as: affordable housing, basic needs, and workforce development opportunities. The team connects people with community-based resources, programs, and services that promote, maintain, and improve individual and community health.

Car Safety Seat Program

The Car Safety Seat program promotes proper safety seat use for children. Free child safety seats and booster seats are available to income-eligible children. Class instruction includes Virginia safety seat laws, safety seat use based on the age and weight of the child, and proper placement of a seat in a vehicle. Classes are available on most Fridays at the Roanoke City Health Department. Please call the health department if you have any questions or to schedule an appointment.

Long Term Care Pre-Admission Screenings

This program provides a timely screening assessment for Medicaid or Medicaid eligible clients needing nursing home placement or services in the home or community. The target population for this program includes the elderly, disabled adults, or children needing a waiver for long-term placement or services for home based care. Screenings are conducted in partnership with the Department of Social Services. A nurse and a social worker screen the client to determine if they meet Medicaid’s criteria for either (1) Functional capacity: the degree of assistance an individual needs to complete the activities of daily living (bathing, toileting, dressing) or (2) Medical or Nursing need (wound care). Services may include the following: in-home care, adult day care, or admission to a care facility. Referrals for screenings may be made through the Roanoke City Health Department.

Substance Use

Roanoke City and Alleghany Health Districts are committed to supporting community health through the prevention and treatment of substance abuse.

  • Overdose Prevention - RCAHD offers education, resources, and information about overdose prevention. We provide REVIVE! Lay Rescuer trainings, Naloxone (overdose-reversing medication), harm reduction test strips, and drug disposal kits.
