Funding Equity and Prioritizing Populations Disproportionately Affected by COVID-19 through Cycle 4 Funding
COVID-19 Vaccination Funding Application for Community Partners
The Prince William Health District is now offering grants to community based nonprofit organizations. The purpose of this grant is to fund strategies that ensure greater equity and access to COVID-19 vaccines by those disproportionately affected by COVID-19.
Your organization may qualify if:
- There is an established history of serving in Prince William County and/or the Cities of Manassas and Manassas Park.
- There is a demonstrated reach into communities disproportionately affected by COVID-19.
- Your organization has experience collaborating with community partners, entities, and organizations to help improve health outcomes.
- Your organization understands and can work within a reimbursement model.
Maximum awards will be $95,000 per organization. *If your organization receives more than $750,000 in federal grant funds, it may be subject to an IRS single audit.

Overview of Application
- COVID-19 Vaccination Supplement 4 was established in April 2021 through the support of the Coronavirus Response and Relief Supplemental Appropriations Act of 2021 and the American Rescue Plan Act of 2021.
- The purpose of Cycle 4 is to fund strategies that ensure greater equity and access to COVID-19 vaccines by those disproportionately affected by COVID. Maximum funding requests may be up to $95,000 for a one-year period.
- Through data driven strategies, the Prince William Health District (PWHD) has identified populations experiencing the heaviest burden of illness and/or the lowest vaccination rates.
- With these targeted populations in mind, the PWHD has created a Community Engagement Branch, which consists of a team that engages in community outreach and expands community partnerships.
- This collaboration with community partners, especially those with broad reach into, and a demonstrated history with populations identified as those disproportionately affected by COVID, will help to achieve the goal to increase vaccination rates and improve health outcomes in the communities in Prince William County and the Cities of Manassas and Manassas Park.
- Funding will be provided through a reimbursement model,
- Funding allocations will be made on a rolling basis and in multiple rounds until Cycle 4 funds are exhausted.
- People in racial and ethnic minority groups (Non-Hispanic American Indian, Alaska Native, Non-Hispanic Black, Hispanic)
- People living in communities with high social vulnerability index
- People living in rural communities
- People who are homebound or isolated
- People who are underinsured or uninsured
- People who are immigrants and/or refugees
- People with transportation limitations
- People with disabilities
- People living in congregate settings
- People with diagnoses of certain underlying health conditions that increase risk of severe disease.
Factors That Affect Your Risk of Getting Very Sick from COVID-19 | CDC
- Financial awards may be provided by the PWHD through the Cycle 4 Supplemental funding to community partners with a plan to ensure greater equity and access to COVID-19 vaccine for those disproportionately affected by COVID.
- Community partners will use funding to implement, support, and/or expand vaccination activities. Maximum funding requests may be up to $95,000 for a one-year period.
- Funding will be provided on a rolling basis and in multiple rounds until Cycle 4 funds are exhausted.
- Activities should focus on vaccine education, awareness, and uptake in populations at higher risk of COVID.
- Sustainability is a key component in the activities funded, with a need for continued and ongoing vaccination efforts for COVID-19 boosters, influenza vaccines, and other seasonal vaccines, as well as pandemic preparedness.
- Funding will be provided through a reimbursement model. Please discuss with the Cycle 4 Partnership Liaison to understand if the reimbursement model for funding will work for your organization.
- Fund efforts to increase equity of health information sharing across communities of focus (for example, funding for staffing of a call center to answer questions about eligibility and scheduling, funding for provision of translation services, etc.).
- Support free or subsidized transportation to access vaccination appointments.
- Fund resource activities to develop tool kits, checklists, and guides aimed at increasing vaccine education, awareness, and uptake.
- Support ongoing training of trusted messengers who address vaccine hesitancy, misinformation, and historical injustices.
- Fund efforts to establish the infrastructure or method to track the vaccination status of individuals living in congregate settings or receiving services through a community organization.
- Funding to address specific social determinants of health that influence populations most severely affected by COVID.
- Implementation of temporary, mobile, or other vaccination clinics during community events or in high-disparity communities.
- Coordination of in-home vaccinations.
- Support increased staffing of medical personnel who administer COVID-19 vaccines, with an emphasis on cultural competence and patient education.
- PWHD will conduct multiple open rounds for accepting applications and issuing awards.
- The Cycle 4 grant period of performance is until April 30, 2025.
- All proposals for round two will be submitted via email to PWHD@vdh.virginia.gov. Please use the subject line “COVID-19 Funding Proposal”.
- The application period for ROUND 2 funding opens Friday, April 5 and ends Friday, April 19. All applications must be received by 11:59pm on April 19.
- Please prepare a proposal that is typed, double-spaced, Times New Roman, 12 font, maximum of 600 words in narrative.
- Describe the projected goal and plan of your organization in using the funding to ensure greater equity and access to COVID-19 vaccines.
- Clearly identify the at-risk population who is targeted in your strategy.
- Please provide an estimated budget. If your organization’s proposal is accepted, additional budgetary details will be developed in collaboration with a Cycle 4 financial representative.
- Successful applicants will be required to create an account on the Virginia supplier portal called eVA. Further instructions will be provided as needed.
- Monthly progress reports will be submitted describing the work done in the past month to meet the projected goals, specifics as to how funding was utilized in achieving the goals, and successes/challenges faced.
- A year-long grant period for applicants is preferable; however, less than a year is negotiable depending upon the project.

- All applications must be received via email by 11:59pm on Friday April 19, 2024.
- A panel review will take place the week of April 22, 2024.
- Awards will be announced the week of April 29, 2024.
- Successful grant applicants will receive instructions for proceeding through the eVA supplier portal. This step will occur on a rolling basis and the timeframe is dependent on the number of awards.

Grant Award Recipient Funding - Serving Prince William County, and the Cities of Manassas and Manassas Park
Youth Under God - Serves low-income communities, unhoused individuals, and isolated individuals in Prince William County and the Cities of Manassas and Manassas Park. Through their Youth Helping Neighbors program they will mobilize adult staff and 28 Youth volunteers to go door-to-door using canvassing efforts as a Mobile Outreach Unit into neighborhoods where transportation is limited, and where needs are greatest. Using a face-to-face approach to build trust, reduce hesitancy, address Covid-19-related health disparities, increase testing among populations at higher risk and those who are under-served, including racial and ethnic minority groups while encouraging ongoing vaccination efforts for Covid-19 boosters, influenza vaccines, and other seasonal vaccines.

Literacy Volunteers of America - Prince William (LVA-PW)
Serving English language learners and through the Power of Holistic Literacy program, LVA-PW will integrate English-based literacy instruction with comprehensive health education, including vaccine awareness, support ongoing training of trusted messengers as tutors who address vaccine hesitancy, misinformation, utilize advanced technologies like AI to adapt vaccine information from sources like the CDC to beginner ESL levels, and implement proven teaching methodologies such as the teach-back method, simplified language, and graphics to make COVID 19 based health information more understandable.

Serving veterans aged 65 and over Hero's Bridge uses a Community Health Worker Model of care and will incorporate COVID related assessment, education, promotion and facilitation into their daily work with Prince William County veterans. This funding will provide increased staffing of established call center and volunteers facilitating transportation and companions to vaccine appointments.

Northern Virginia Family Services
Serving at-risk populations, NVFS will deliver referral services, parent education sessions on pediatric care topics including COVID-19 vaccination, focus on sustainable activities that meets the need for continued and ongoing vaccination efforts for COVID-19 and other vaccines, train trusted messengers within programs about the importance of COVID 19 vaccination uptake as well as other childhood vaccinations

Serving adult English language learners and through their Healthy Students program model will address barriers to access of healthcare services among low-income immigrants with low-English proficiency, develop educational materials including guides and visual aids using medical terminology/phrases, integrate digital literacy tools, and design/launch a comprehensive information hub with a focus on health education, COVID-19 vaccination and testing, and wellness topics with an ability to translate to 90 or more language options.
