OCME Records Request Form for Legal Next of Kin
The legal next of kin of a decedent may submit an OCME record request using the OCME Records Request Form for Legal Next of Kin. Please mail the request form and the fee to the appropriate OCME District Office. Only certified funds shall be accepted (certified check, money order, cashier’s check). Checks and money orders should be made payable to Virginia State Department of Health.
