Information regarding fees for health department services can be found below. Services that we normally provide will not be denied because of the inability to pay. All private health information will be kept in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
- What Do I Bring?
When you come to a clinic, you may need to bring:
Proof of Identity, acceptable forms include
– Social Security card
– Driver’s License
– Medicaid card
– For an infant or child, acceptable forms include: hospital bracelet, foot prints, hospital crib card, birth certificate, Social Security card, Medicaid card, Income tax return or W2 Form
- Proof of Income, acceptable forms include
– Pay stub showing year-to-date 3 months worth of pay stubs
– Income tax return or W-2 form
– Medicaid card
– Written statement from employer
– Support letter (letter showing that someone provides you with food and shelter)
– Statement of Social Security of income received
– Statement from employment office of unemployment or of benefits you are receiving
- Medicaid card
- Health insurance card
No-Cost Services
The following services are offered by the health department at no out-of-pocket cost to client.
- Blood pressure screening
- Nutritional counseling
- Nursing Home/Personal Care screenings
- School required immunizations for all children
- Skin testing for those who are contacts of a suspected or confirmed tuberculosis patient
- Tetanus/Diphtheria immunization for adults (vaccine is free, but there are administrative fees charged)
- Car Seat training (Medicaid required)
- Permitting of sewage disposal system repairs
- Investigation of animal bites/rabies control
- Investigation of:
– Communicable diseases
– Communicable disease outbreaks
– Sewage disposal system complaints
– Food establishment complaints
Sliding Fee / Flat Fee Services
Costs for the following services will be determined by the health department’s eligibility process.
Flat Fee
- Flu and pneumonia shots (unless patient has Medicare or Medicaid to pay for this service)
- PPD (TB skin testing)
- Sewage disposal and well permits
- Restaurant and tourist establishment permits
- Birth and death certificates
- Pregnancy testing
Sliding Fee
- Maternity Clinic
- Family Planning & birth control
- Tuberculosis medications
- School Physicals
- Testing of sexually transmitted infections based on condition, may be chargeable and/or billed to insurance
