2023 Community Health Assessments

Community members attend an educational program

A Community Health Assessment (CHA) is a thorough evaluation performed at various levels, including state, tribal, local, or territorial jurisdictions. This assessment identifies significant health needs and issues within the community by engaging in a systematic process of collecting and analyzing detailed data from various sources. The objective is to gain a comprehensive understanding of the community's health status and the factors influencing it.

Based on the findings from the CHA, a Community Health Improvement Plan (CHIP) is crafted. This plan outlines strategic actions and initiatives designed to address the identified health issues and improve overall community health outcomes. The CHIP relies on the insights gained from the assessment and incorporates input from the community to create actionable steps for enhancing health and well-being.
