Community Health Worker (CHW)
Hiring Range: $20-$24/hour
Status: Grant-funded contract position, available immediately.  
Work Schedule: Monday-Friday, up to 40 hrs/week; some evenings and weekends
Grant Period: Now through June 30, 2025; may be extended depending on continued funding.    

Job Description, Duties & Responsibilities: The Chickahominy Health District (CHD) has been awarded funds which can be used to place Community Health Workers (CHWs) with non-profit organizations. Thrive Virginia, a non-profit community action agency, has been selected by CHD for CHW placement. The CHW will work up to 40hrs/week at Thrive Virginia completing the projects described below and as a CHD contractor. 

Thrive Virginia is dedicated to supporting and uplifting communities by using a Whole Family Approach which involves identifying factors that undermine a family’s overall well-being, then working with the family to solve problems, access new resources, and sharpen existing skills so they are better prepared to overcome barriers to their success. The CHW will provide direct services to children and their families who are impacted by health-related social needs. The work will include developing programming to improve access to healthcare resources and facilitating health education activities and workshops.  An emphasis will be placed on educating the public about staying current with CDC recommended immunizations.   

Position Activities:   

  • Convene a work group of community partners and Lived Experience (LEx) Leaders from the designated communities to assess and address relevant health equity topics.   
  • Work with Thrive Virginia’s Health Specialist to develop a series of health lessons that focus on healthy living topics for under-resourced families, i.e., navigating Medicaid; finding providers; immunizations and vaccines; SNAP; and WIC Recipes.   
  • Implement a schedule for individual sessions and ongoing group workshops that would be accessible to members of each designated community.  This will include in-home, in the community, or virtual options.     
  • Cultivate relationships with local health providers to advocate for participants, including primary care physicians, pediatricians, and other health care professionals.  This includes identifying ways to minimize barriers to scheduling and keeping appointments.   
  • Develop a campaign about the importance of immunizations and vaccinations, including evidence-based resources and professional guest speakers.   
  • Promote awareness of local health department programs and services  
  • Develop a comprehensive report including data collected detailing the work completed and “lessons learned”.   
  • Up to 20% of the position will include travel.  

Minimum Qualifications:   

  • Bachelor’s degree from an accredited college, or equivalent training and experience.   
  • At least one year of professional experience in health education, human services, social work, healthcare, or a related field. 
  • Ability to prepare reports and communicate effectively, in oral and written form.  
  • The ability to analyze facts and exercise sound judgement when solving problems within the scope of responsibility.  
  • Highly organized and self-motivated to work both independently and collaboratively in teams.  
  • Ability to be comfortable and sensitive working with community members in a variety of settings and with culturally diverse populations.  
  • Ability to establish and maintain effective working relationships with families, social services, community partners, and the public.  
  • General knowledge of the use of standard office equipment and associated software. 
  • Ability to liaise between the community, Thrive Virginia, and the Chickahominy Health District to raise community awareness of programs and services available. 
  • Possess and maintain a valid State driver’s license and the ability to drive organization owned/personal vehicle to community events. Work related mileage can be reimbursed at the federal rate. 

Preferred Qualifications:   

  • Be proficient in system navigation, including how to apply for public benefits like Medicaid, SNAP, WIC, TANF, and Social Security.    
  • Experience in providing services to culturally diverse communities and families, a strong background in prevention services to families and children, including health assessments, education, and goal planning.    
  • Experience in health education outreach especially related to overcoming access barriers, the promotion of CDC recommended vaccinations, and mitigating the spread of misinformation.   
  • General knowledge of marketing techniques.   
  • General knowledge of community and agency resources and programs.   
  • Knowledge of nutrition guidelines and immunization recommendations, and experience educating the public about such topics.    
  • Special consideration to persons who can speak English and Spanish fluently.
