Child and Adult Waiver Screenings- Uniform Assessment Instrument (UAI)

Long Term Services and Supports Screenings (pre-admission screenings) are required for any person requesting:
  • care in a Medicaid certified facility.
  • in-home care who are eligible and approved for Medicaid.

A detailed assessment is performed by a public health nurse and a family service worker to determine need and appropriate setting for care. Based on the assessment and recommendations for care, the individual and their family are given information to contact services available to them.

  • Chesterfield or Colonial Heights: 804-748-1100, select option 5, and ask for Adult Services.
  • Powhatan: 804-598-5630.

A person helps a teen in a wheelchair.

To schedule an appointment for a child residing in:

  • Chesterfield: 804-748-1691 and ask for a pediatric screening.
  • Colonial Heights: 804-520-9380.
  • Powhatan: 804-598-5680.

Please note that any DSM-5 (Diagnostic and Statistical Manual) diagnosis alone (including autism) may not qualify for the medical criteria needed for this waiver.
