Long Term Services Support (LTSS)

WHAT IS LONG TERM SERVICES SUPPORT (LTSS)?

The LTSS program is a Medicaid-funded program covering in-home care, PACE (Program of All-Inclusive Care for the Elderly), adult day care and nursing homes.  Program participants are able to use one or multiple of the available services.  LTSS’s goal is to keep program participants in their homes as long as possible.

WHO IS ELIGIBLE?

Anyone is eligible for the LTSS program as long as they are at a nursing home-level of care.  Participants must meet one of the following requirements: need assistance for functional daily living activities, have an ongoing medical nursing need, or be in need of or at risk of needing to be placed in a nursing home within 30 days.

Anyone can request a screening for themselves or for a family member or friend, but the individual must be willing to complete the screening.

IS THERE A COST?

There is no cost for the initial screening, but an eligible participant must be enrolled in Medicaid to participate in the program.

If you are making a referral for a child, please call the Central Virginia Health District at (434)-477-5900.

If you are making a referral for an adult, contact your local Department of Social Services office for a screening.
