Arthritis Health Care Provider Toolkit

This Health Care Providers Toolkit includes:

1. Information about arthritis conditions who do not have formal medical training (i.e. community health workers),
2. Scientific background on physical activity assessment and counseling using evidence-based tools,
3. Information on Arthritis-Appropriate-Evidenced-Based-Intervention (AAEBIs) that VDH supports through grant funds, and
4. Encourage referrals through platforms including: Unite Us and Find Help.
